It is difficult to get the collar snug enough to get the full effect without choking yourself. It works well if you lay on it as it provides a large surface of cold but not so great for wearing while sitting up. I didn't try the heat, just the cold.
These heat packs are comfortable, warm well in the microwave and are comfortable as well as lasting heat through the session.
